What is Support Coordination

The NDIS empowers you to decide which supports, services and providers you use; however, sometimes coordinating supports can be tricky. 

That’s where a Support Coordinator comes in.

A Support Coordinator helps you manage your NDIS plan, whether it’s for a short-term or ongoing period. They assist you in organising your support, building your skills, and achieving greater independence.

Additionally, they help you actively participate in your local community and work towards your goals. With the help of a Support Coordinator, you can get the most out of your NDIS plan while enjoying greater freedom and independence.

How we can support you

A Support Coordinator can help with:  

  • Understanding your NDIS plan. 
  • Connecting you to supports and service providers.  
  • Setting up service agreements with your NDIS providers 
  • Planning your schedule.     
  • Accessing community services and activities, including learning or employment opportunities, health and accommodation services, and community activities.  
  • Liaising with the NDIS when needed.  
  • Supporting you through changes or NDIS plan reviews. 
  • Building your confidence, so you can coordinate your own supports in the future. 

We want to ensure that your NDIS plan works for you to help you live the life you choose.  

Who can access Support Coordination

If Support Coordination is deemed a reasonable and necessary support for you, it will be included under ‘Capacity Building’. 

If your plan references the level of support coordination funded, you can only purchase that particular level of support coordination. If your plan does not describe the level of support coordination funded, you can decide which level of support coordination best suits you.

Support Coordinators are independent so you can be sure that their only job is to help you look at your options and find the best provider and supports for you.
